Your health and well-being is important to us!
LIVESMART is a wellness program designed to improve the health of O’Neal employees and spouses. We encourage all employees and spouses to participate in the annual screening and coaching events and enjoy the benefits of the program. (see benefits here)
WHAT DOES SCREENING INVOLVE?
You can complete the health screening during the on-site screening event at your spouse's work location or you can complete it with your health care provider. More details about each option can be found below. We will be obtaining the following information during your screening:
blood work, height, weight, blood pressure, waist circumference.
WHAT DOES HEALTH COACHING LOOK LIKE?
A health coach will schedule a private health coaching session (up to 20 minutes) where you
develop a plan of action for your health goals, focusing on self-care, exercise, & nutrition.
HOW DOES IT WORK?
The program begins by completing your health screening. This can be completed with your health care provider or at your spouse's work location at one of the health screening events throughout the year.
If you would like to complete with your health care provider, take the Qualification Form to your appointment. Mail or Fax the completed form to LIVESMART.

STEP 1: ENGAGE

Once you complete your health screening you will connect with your personal health coach to review your results and inform you of your Rewards Club Level (Platinum, Gold, Silver, Bronze). You will connect with a health coach for an initial coaching session & up to two follow-up sessions throughout the year depending
on your results.
Achieve your goals and succeed in your health journey by participating with your spouse in up to two program campaigns each year. See the LIVESMART brochure or website for more details about the program campaigns and resources available.

HOW DO I GET STARTED?
OPTION 1
Attend an on-site screening
and coaching event.
2. Contact your location ambassador or call LIVESMART to sign up for the next on-site screening and coaching event.
OPTION 2
Complete Your Screening with
Your Health Care Provider.
1. Download the Medical Qualification Form
2. Schedule & complete an appointment with your health care provider who will fax your form to LIVESMART
3. We will contact you once your form is received.
