LIVESMART PORTAL
The LIVESMART Portal is your link to access your LIVESMART Health screening results, complete your online personal health assessment (PHA), complete campaigns, track health behaviors, and gain access to additional recipes and health information. This portal has an increased level of privacy and security to maintain confidentiality of health information.
After registering your account online, download the portal app on your smartphone by searching "WellSteps" in your app store.
STAY ENGAGED!
We encourage you to stay engaged with the LIVESMART portal on a regular basis by logging into your account online or using the app. You will be unregistered if your account goes inactive for more than 3 months. If this occurs, please complete the steps to register and log back in. This will not cause a loss of data or information in your account.
